Please fill out the form below, acknowledge the Terms and Conditions and make sure to include your RCPV account number, if applicable. This will be used to track your points during the contest between June 1st, 2022, to July 31st, 2022.

Eligible entrants must be Eye Care Professionals located within the United States.
